Experiences of the Hydroflex Penile Prosthetic Surgery

Abstract
Ten cases of the Hydroflex penile prosthesis (a new type of inflatable prosthesis), which is self-contained and totally intracorporeal, were implanted in 9 patients of erectile impotence at the Department of Urology, Chung-Ang University Hospital between October l985 and July l986. The results were obtained as follows: l. The average patient age was 42.7 years, with a range of 23 to 63 years. 2. The causes of erectile impotence were organic in 8 cases (88.9%) and psychogenic in 1 case (11.1 %). Four cases (44.5%) were neurogenic and traumatic patients who had the accompanying urethral stricture in 1, neurogenic bladder in 1, true incontinence in 2 patients who underwent simultaneous implantation of the artificial sphincters. One traumatic patient underwent 3 operations because of surgical complications. 3. The size of the Hydroflex ranged from 13cm to 21cm of cylinder including rear tip extenders (R TE) in length. Out of 1O cases 5 were 18cm (3 cases) or 19cm (2 cases). The size of the Hydroflex was all 11mm in diameter. 4. The Hydroflex penile prosthesis was implanted mostly (80.0%) under local anesthesia and mostly (90.0%) through penoscrotal incision. Average operation time was 46 minutes. 5. Duration of the post-operative admission was mostly (80.0%) 6-9 days. There were 4 case of complications. Three cases of surgical complications were disruption of surgical wound in 1, iatrogenic puncture of the prosthesis during closing cavernosotomy in 1 case Another 1 case was bowing of the glans which was caused by rupture of the corporeal crus during dilatation of corpus cavernosum and going out of the prosthesis from the corporeal body through the ruptured crus. Mecgnical complication, fluid leak developed in 1 case In conclusion, the Hydroflex penile prosthesis is not difficult to implant technically And we think that the implantation of the Hydroflex penile prosthesis is an effective method for the treatment of erectile impotence, through its mechanical reliability should be said after long term follow-up.
